The Shared Care Clinical Outreach Program

This program, in partnership the University Health Network, works with men and women who are homeless, under-housed, and experiencing serious and persistent mental health concerns. Shared Care Clinical Outreach Service addresses the physical and mental health needs of homeless people using the shelters, hostels and drop-ins in the downtown Toronto area. Team members include full-time registered nurses, outreach workers, visiting family physicians and visiting psychiatrists as consultants.
